Complete the following information about your child:

  • Full name (as per school records)
  • Date of birth
  • Gender
  • Class/Grade
  • School name and address
  • UDISE code of the school (ask your school administration if you don’t know this)

Add following details:

  • Parent/Guardian’s full name
  • Relationship to the student (mother, father, legal guardian)
  • Mobile number
  • Email address (if available)
  • Full residential address

Read all consent declarations:

  • What information will be stored in the APAAR system.

Check all term and complete next step:

  • Sign in signature box and write your full name.
  • Enter the current date.
  • For verification add your mobile number.

Note

Only parent or legal guardian signed the form. Signatures by students, relatives, siblings are not acceptable.
